Dear Parent/Guardian,                

Thank you for allowing me the opportunity to fingerprint and photo I.D. your child.
I felt it was important to explain the importance of this bio card to you.

I recommend that you always keep an updated/current photo of you child with this card.A current picture is crucial to police when searching for a missing child!
Please update every 6 months.

I recommend that you fill all information in pencil. As your child grows, information will need to be updated to match the current picture. Include any information under the distinguishing marks section you feel would be helpful for the police, such as: Has pierced ears, a certain birthmark, wears braces, glasses, etc.

On the back of the bio card, please have your dentist fill in the dental chart section. Ask for a run out of your child’s dental records on a yearly basis, and keep with bio card.

Remember to do the DNA swab on your child. This is extremely important! Follow the easy steps listed.
 
REMEMBER THAT EVERY SECOND COUNTS WHEN SEARCHING FOR A CHILD.  ALL COMPLETED INFORMATION IS VITAL TO POLICE!

I recommend that you keep this card in a safe place in your home. Remember to tell a friend or relative where you have stored it, so that they may be able to retrieve it to give to the police if you are unavailable. Also, feel free to make copies of your child’s bio card for vacation trips, visits to relatives, school field trips, Grandparents, babysitters. Remember to give your child’s school a copy to keep on file.
